I've never seen a copy of Dive Like a Pro either and have had many a time that it was suppose to come with it. Personally it wasn't worth the hassle.
It may also be one of those limited time offers...

Some like to take the class and it isn't a bad idea. Personally I didn't. I dove with a friend that had dove a dry suit for many years and he ran me through it.
Some will teach you to use the drysuit...

Someone also makes a camera and a monitor all in one. So you wouldn't have to add the monitor as it is in this link. I think that the kit was about $200 so that would up your cost. Pretty cool either...

For my OW the shop did it but then for AOW they didn't. I just scanned my drivers license and cropped the pic to fit what PADI wanted. Worked just fine.

Well it looks like they are rather relaxed in Coz or I have only ran into dive shops that are strict. I just did a boat dive in the Great Lakes and they required at least AOW with at least a 100'...

They aren't fun at all. I friend of mine had one in a tooth, or at least the tooth area. She said that she would rather had been shot. They do goa way but it is something that I could do without.
